Managed Health Care Plans

Health Maintenance Organizations (HMO)
* Primary care provider acts as a "gatekeeper" to other healthcare providers
* Only covers specific healthcare providers and facilities

Preferred Provider Organizations (PPO)
* Based on a network of healthcare providers
* May obtain healthcare from providers outside the network
* Offers benefits as financial incentives to use network providers (may include lower deductibles, lower copayments and higher reimbursements)

Exclusive Provider Organizations (EPO)
* Based on a network of healthcare providers
* Does not cover for providers outside network

Point-of-Service Providers (POS)
* Primary care provider acts as "gatekeeper" to services within a network of providers
* May obtain healthcare from providers outside the primary network
* Offers benefits as financial incetives to use network provide

Indemnity Insurance Plans Unrestricted choice of provider; claim forms to file; usually more expensive.
