Submission Form

Every article and/or image submitted to JPHAS for publication consideration must include a completed submission form. All writers, artists, and photographers must complete a submission form for each article and/or image they contributed and/or created. Articles and images will not be considered for publication without this form. Articles including live sources must obtain source approval documentation.

Source Approval Form

The source approval form must be completed for each live source used in or used to supplement an article. Live sources include any person interviewed or photographed for any article submitted to JPHAS. A source approval form for each live source must be included with a completed submission form to be considered for publication in JPHAS.

The source approval form must be used to receive and verify approval for publishing:

1. An individual's statement directly quoted in the article.
2. An individual's opinions and/or thoughts that were noted, referenced, and/or restated in the article.
3. A photograph of the individual with the article.
